How does the acne and cancer drug (Ro)accutane work chemically?
[ ]Section 1: Significant irreversible hormonal antagonism
(Ro)accutane has a suppressive effect on the steroidogenesis (inhibition of formation of hormones, mainly through inhibition of several pathways in the metabolism of cholesterol), supresses hormonal receptors and cellular uptake. Hormones which action is suppressed include thyroid hormone, growth hormone, testosterone, dihydrotestosterone, progesterone, vitamin D and vitamin A. Circulating hormones which also exist in the brain as neurosteroids, with corresponding receptors located in several parts of the brain.

[ ]Section 2: Affected neurotransmitters and parts of the brain
(Ro)accutane interacts with receptors of several neurotransmitters that are g-protein coupled, including corresponding hormonal neurosteroids, serotonin, dopamine, norephinephrine, melatonin and GABA. Malfunctioning receptors, decreased receptor expression and lessened metabolism of neurotransmitters are suggested to be effects of a (Ro)accutane exposure. The retinoid signaling found in several parts of the human brain is affected. A significant fall in metabolism has been observed. Cell losses have been suggested. The affected parts of the brain include the hippocampus, orbitofrontal cortex, blood-brain barrier, the subventricular zone and hypothalamus

[ ]Section 3: Programmed cell death (apoptosis)
(Ro)accutane in concentrations that acne-patients are exposed to (>1 microM) results in excessive loss of cells, in several parts of the body and brain, mainly where retinoic acid receptors RARs and RXRs are expressed

[ ]Section 4: (Ro)accutane and the immune-defense
(Ro)accutane irreversibly alters the immune-defense in several ways, by altering cytokine production and more. Vitamin A is extensively linked to immune-function. The innate immune system, or the function of several of the receptors that recognize and initiate immune responses are suggested to be significantly altered.

[ ]Section 5: (Side)-effects in subjects exposed to retinoic acid and their suggested causes (A-Z) (1 sub-board)
(Ro)accutane is a non- selective, non-reversible toxic acne-"treatment" and therefore has several very severe effects. (Side)-effects ordered alphabetically: Page 1: A-G Page 2: H-S Page 3: T-W and other (side)-effects. List not complete.

[ ]Section 6: (Ro)accutane effects suggested to worsen with age
Adolescents is an age group exposed to (Ro)accutane. This during a hormonal peak that occurs. There are several age-related changes in the body and brain. A decline in growth hormone, androgens and other hormones occurs with age. Receptor expression in cells change and so does metabolic activity. Insulin sensitivity changes with age. The long-term effects from (Ro)accutane exposure are unknown. The effects of a (Ro)accutane exposure correlate with the theories that by the scientifical community as a whole, at present are held as an explanation for the development of among other things alzheimers disease, dementia and cognitive decline.

[ ]Section 7: Gene-expression
(Ro)accutane, 13-cis-retinoic acid, is a derivate of vitamin A, which although being though of as a vitamin, has very little in common with other vitamins except for vitamin D. In practise vitamin A is a hormone, and is therefore classified as a steroid and inserts its effects through a major influence of gene-expression. Section classifying (Ro)accutane induced transcription by transcription factors and enzymes.
